Current Outstanding Balance — approximately how much remains.
Payment Amount — daily or weekly payment amount.
Payment Frequency — daily, weekly, or other.
Remaining Payments — estimated number of payments remaining.
Funding Date — when the advance was funded.
Payment History — recent account statements showing payments made.
ACH Authorization — information regarding automatic withdrawals from the business bank account.
Personal Guarantee — whether the owner personally guaranteed the obligation.
UCC Filing Information — if applicable.
Default Notices or Collection Letters — if the MCA has declared or threatened a default.
Any Modification or Restructuring Agreements — if the original MCA has already been modified.
